PPTA’s room-block is intended for registered attendees of the Plasma Protein Forum only. PPTA withholds the right to remove anyone from the room block who is not registered for the Forum.

Registration

Registration for the Forum includes access to all general sessions, the exhibit hall and all Forum breaks (you must be registered in order to participate in any of the Forum activities). Individuals not registered for the Forum will be given the opportunity to register on-site or will not be granted access to these events. Forum registrants are invited to attend the 25th Anniversary Black-tie Gala, to be held the evening of June 13 at the Andrew Mellon Auditorium. You may register for the Forum and Gala using the same registration form. Please note, Gala attendance is on a first-come, first-served basis and is not guaranteed until a confirmation email is sent from PPTA.

Payment MUST BE received by MAY 1, 2017 in order to receive the discounted rate.
REGISTRATION TYPE:
PPTA reserves the right to confirm registration type and charge appropriately.
SUBSTITUTIONS:
Substitutions WILL NOT be allowed on-site; notification of substitutions must be received by MAY 31, 2017.
CANCELLATIONS:
Member and Non-Member cancellations must be received in writing by MAY 31, 2017 and will be subject to an administrative fee of $250.
PHOTO RELEASE:
By registering for this event, you agree to allow PPTA to use any images taken of you.

GALA ATTENDANCE:
In order to attend the 25th Anniversary Gala, you must register for the Plasma Protein Forum. Subject to availability. Limit ONE guest per registrant.
